This human milk fortifier is formulated for preterm and low-birth-weight infants as a nutritional supplement to be added to human milk. Proper hygiene, handling and storage are important when preparing fortified human milk. Failure to follow these directions could result in severe harm.
- Pour desired quantity of human milk into sterile feeding container.
- Product separation is normal. Massage contents until uniform.
- To open packet, tear top off completely along perforation.
- Add fortifier to human milk and gently shake until mixed well.
- Use or refrigerate immediately.
- Bottle feeding: Discard 1 hour after feeding begins.
- Tube feeding: Limit hang time to a maximum of 4 hours.
- Use immediately or store unused fortified human milk in the refrigerator; use within 24 hours or according to hospital procedures for handling human milk.
- After refrigeration, remix gently if needed and feed promptly.
- Store unopened packets at room temperature in this protective carton; avoid extreme temperatures.

This human milk fortifier is formulated for preterm and low-birth-weight infants as a nutritional supplement to be added to human milk. Proper hygiene, handling and storage are important when preparing fortified human milk. Failure to follow these directions could result in severe harm.
- Pour desired quantity of human milk into sterile feeding container.
- Product separation is normal. Massage contents until uniform.
- To open packet, tear top off completely along perforation.
- Add fortifier to human milk and gently shake until mixed well.
- Use or refrigerate immediately.
- Bottle feeding: Discard 1 hour after feeding begins.
- Tube feeding: Limit hang time to a maximum of 4 hours.
- Use immediately or store unused fortified human milk in the refrigerator; use within 24 hours or according to hospital procedures for handling human milk.
- After refrigeration, remix gently if needed and feed promptly.
- Store unopened packets at room temperature in this protective carton; avoid extreme temperatures.

PRECAUTIONS
- This product is nutritionally incomplete by itself and is designed to be added to human milk.
- Add only to human milk - do not add water.
- Never use a microwave oven to warm fortified human milk; serious burns can result. Test temperature before feeding to avoid scalding the baby’s mouth.
- Follow label instructions when using this product. Abbott has not evaluated this product to fortify human milk in a clinical trial at energy densities greater than 24 Cal/30 mL (81 Cal/100 mL). Therefore, the occasional use of a higher energy density should be done under strict medical supervision.
- Additional iron may be necessary.
- Tolerance to enteral feedings should be confirmed by offering small volumes of unfortified human milk.
- Once enteral feeding is well established, Similac® Human Milk Fortifier Extensively Hydrolyzed Protein Concentrated Liquid can be added to human milk.
- Not intended for feeding low-birth-weight infants after they reach a weight of 3600 g (approximately 8 lb) or as directed by a health care professional.

ADD ONLY TO HUMAN MILK. DO NOT ADD WATER.
This human milk fortifier is formulated for preterm and low-birth-weight infants as a nutritional supplement to be added to human milk. Proper hygiene, handling and storage are important when preparing fortified human milk. Failure to follow these directions could result in severe harm.
- Pour desired quantity of human milk into sterile feeding container.
- Product separation is normal. Massage contents until uniform.
- To open packet, tear top off completely along perforation.
- Add fortifier to human milk and gently shake until mixed well.
- Use or refrigerate immediately.
- Bottle feeding: Discard 1 hour after feeding begins.
- Tube feeding: Limit hang time to a maximum of 4 hours.
- Use immediately or store unused fortified human milk in the refrigerator; use within 24 hours or according to hospital procedures for handling human milk.
- After refrigeration, remix gently if needed and feed promptly.
- Store unopened packets at room temperature in this protective carton; avoid extreme temperatures.
